New Patients at Smith Wellness
When you book your appointment at Smith Wellness, you can complete your new patient paperwork online to save yourself some time in our office. However, if you’d prefer to complete the paperwork here, that’s perfectly okay! Please bring your driver’s license or government-issued identification and your insurance card. If you’ve had recent imaging done, you can bring that as well.
Your First Visit
When you come in, our cheery front desk staff will greet you by name and help you with any paperwork or insurance questions. Then our tech will bring you back to go over your health history and take your vitals.
Dr. Smith will come in and talk with you about your current condition and any issues in your history that may be contributing. He’ll take the time to truly listen to your concerns, explain everything he’s doing, and complete a thorough examination. If X-rays are necessary, he will refer you out for those. If it’s safe and appropriate to do so, he will adjust you at this first visit. You may also receive any healing therapies indicated for your situation, such as massage, e-stim, ultrasound or dry needling.
Your Second Visit
At this visit, Dr. Smith will talk with you about how your body has responded since your first adjustment. He’ll outline his plan for your ongoing care, and then you’ll receive your adjustment and therapies.
Financials
We do our best to verify your insurance coverage before you arrive for your first visit, so we can explain your benefits and costs before we begin. We are in-network with most insurance plans, including Medicare. We are also a Community Care Provider for the VA.
You may also use ChiroHealthUSA or your HSA or FSA to pay for your care, and we offer payment plans.
